TVCC staff members are unable to share information from your academic or financial records with anyone (if you are under 18 years of age, special rules may apply) without your written consent. Dependent/Independent Verification Form
We use this form to collect information about your family size, how many of your family are in college, and miscellaneous untaxed income that normally would not appear on your taxes or W-2's. Be sure to fill out this form completely - leave nothing blank - use zero's or n/a's if a question does not apply to you.

Document Your Financial Support
The information on your FAFSA suggests that your (or your family's) combined income was unusually low for the number of people in your family. We use this form do document non-traditional forms of support and income such as how rent/mortgage, utilities, grocery, and transportation expenses were paid, etc.

Application for Admission
TVCC Financial Aid uses data from your admissions file to cross-check and confirm your identity and the degree you are seeking. Submit a completed Application for Admission with your current information to the Admissions Office.

Financial Aid Distribution Authorization and Direct Deposit
TVCC can now deposit your financial aid refund check (if applicable) directly into your checking or savings account. If you would like to sign up for direct deposit, you must complete BOTH the Financial Aid Distribution Authorization and the Direct Deposit Authorization and submit them to the TVCC Business Office.

Change of Major
In order to receive Federal financial aid you must be pursuing a degree that qualifies for aid. Use this form to make sure we have your correct major/degree.

Early Aid Advance Application
TVCC will disburse financial aid to student accounts on the third Tuesday of each term. For students whose financial aid exceeds their instutional charges (tuition and fees) up to $500 of the anticipated excess funds can be requested in advance (on or after the first day of the term.)

Student/Spouse Federal Tax Forms
Submit a copy of your Federal 1040, 1040A, 1040EZ, or 1040X. Do not submit copies of schedules or worksheets unless specifically requested. Lost your Federal Tax forms? Contact the Internal Revenue Service at 1-800-829-1040. Select option #1, then option #4 and ask for a "tax return transcript."

Student/Spouse W-2's
Submit copies of W-2's from all employers in the year requested. If you had income from other sources that was not reported on a W-2, provide alternative documentation of income (such as a 1099 or Social Security Benefits Statement). Lost your W-2's or 1099's? Contact the Internal Revenue Service at 1-800-829-1040. Select option #1, then option #4 and ask for a "W-2 & 1099 transcript."

Clarify Your Current Degree Program
Submit a written statement clarifying exactly what degree you are seeking from TVCC. Students commonly indicate on the FAFSA that they are seeking a Bachelor's degree. (TVCC offers Associate degrees, Associate of Science degrees, Associate of Applied Science degrees, and certificate programs.)

Clarify Your Current Grade Level
Submit a written statement clarifying your current college grade level. Students commonly leave this question blank on the FAFSA or, indicate they are a Junior, Senior, or Graduate student. Your grade level is assigned according to the number of credits you have earned or transferred to TVCC. If you have less than 45 earned credits, you are in your 1st academic year of college and are a freshman. If you have greater than 45 credits, you are in your second academic year and are a sophomore. You are not considered a junior until you transfer to a four-year college or university.

Clarify FAFSA Worksheet "A", "B", or "C"
You reported a dollar amount on your FAFSA application in one of these worksheets that we could not verify using your tax and W-2 information. Submit a written description of the amount your originally reported on the FAFSA.

Graduation Plan
Your TVCC transcript indicates that you are approaching graduation or have enough credits to graduate already. Make an appointment with an academic advisor to discuss the classes you need to take in order to fulfill your degree requirements. Submit a copy (signed by your academic advisor) of the graduation plan you and your advisor discuss.

Official Academic Transcripts
Submit official transcripts from all college, university, or post-secondary schools you have attended.

Academic Transcript Evaluation
Your official academic transcripts will be evaluated for transferable credits. The academic advising staff will provide TVCC Financial Aid with a transfer credit report. You may be asked to submit a graduation plan pending the outcome of your academic transcript evaluation.

Review & Correct FAFSA Application
There are significant issues with your FAFSA application that prevent processing. Review your SAR and make corrections to incorrect or missing data.

Resolve NSLDS Issue
The U.S. Department of Education has indicated that you have a defaulted student loan or owe a repayment of federal financial aid funds. Contact your loan servicer or previous school immediately to resolve the issue.

Documentation of Citizenship Status
Your status as a U.S. citizen or eligible non-citizen could not be confirmed by the INS or Department of Homeland Security. Bring your original documentation to the TVCC Financial Aid office or a distance center. A student services representative will examine your documentation for authenticity and make a copy for your financial aid file. Do not make a copy yourself.

Document SSI, Food Stamp, Free/Reduced Lunch, TANF/TAFI, or WIC Benefit
You indicated on your FAFSA application that you received one of the above benefits in the past year. However, other income information on your FAFSA indicates that you may not be eligible for the benefits you reported receiving. Submit documentation from the agency providing the benefit that summarizes the benefit amount for the year in question.

Appeal Forms

Please review TVCC's Financial Aid Standards of Satisfactory Academic Progress before beginning the appeals process.

Financial Aid Suspension Appeal
Students who have not met the Standards of Satisfactory progress can appeal to a faculty committee to reinstate financial aid eligibility. This appeal must be submitted with a signed graduation plan from your academic advisor. TVCC Financial Aid will not comment on the progress of an appeal until the appeals committee notifies us of their decision.

Financial Aid Appeal to Complete an Additional Degree/Certificate
Students who have already earned an Associate Degree, one or two year certificate, or Associate of Applied Science may appeal to complete a second degree/certificate. This appeal must be submitted with a signed graduation plan and transcript evaluation from your academic advisor. TVCC Financial Aid will not comment on the progress of an appeal until the appeals committee notifies us of their decision.

Financial Aid Special Circumstances Appeal
This form is intended for students requesting a recalculation of financial need based on special or unusual circumstances.
